Why we need this

Before your profile goes live, we need one IRS tax form on file for every tutor. Ms.Tutor LLC is registered in the United States, so this is required for paying tutors.

How to send it back

1. Open your form and fill it in on your computer or phone.
2. Sign it, then save it as a PDF or take a clear photo.
3. Email it to hello@mstutor.academy with your name in the subject line.
4. We will confirm once your profile is activated.
